1. Make sure to print application.
2. Make sure that correct social security number and birth date is on application.
3. Make sure the correct number of people in the party is written on app, and all applications are stapled together (if applying as a party).
4. If born after January 1, 1960 you must attach proof of hunter education card.
5. Make sure to fill out and sign Power of Attorney in the presents of a notarial official.
6. Make sure correct credit card type, # ,and expiration date are on app
7. Only one credit card # is needed for a group (Please remember this card will be processed for the entire groups tag fees, if tags are drawn).
8. Make sure to enclose deposit of $400.00 on hunt to Mitch Buzzetti .
9. Make sure to Sign and Date application.
10. Make sure legal guardian or parent signs application for any hunter under 18 years of age.
